Each Attendee Must Complete a Registration Form

All registration must be received in the CPCO office by September 30, 2025. A $25 fee will be charged for onsite registration. Cancellation/Substitution:If you can’t attend but can send a substitute representative, just phone our office. If you need to cancel, please phone by 9/30/25 so that we may process a refund for you. Cancellations after this date to 10/6/26 will receive a refund less 25%. All refunds to credit cards will incur a 3% processing fee.
No refunds for no shows or cancellations received after 10/6/25.
